  • What should I bring to my first appointment?
    • New Patient Registration Forms (please refer to Patients Forms – upper right)
    • Referral/prescription/script (if you have already seen your health care provider about your condition)
    • Health insurance card (if you prefer that we bill your health insurance carrier for your physical therapy services) 
    • Operative reports, if applicable and available
    • Imaging results (e.g. MRI, Xray, EMG, etc.), if applicable and available
    • Equipment you may be using for this condition (e.g. brace, splint, crutches, cane, walker, etc.)
    • Appropriate clothing (please see “What should I wear” – below)

  • How long will my appointment take?
    • You will be scheduled with your physical therapist for approximately one hour on the first day. The length of your treatment sessions will be determined on a case-by-case basis based on your unique situation and needs, but in general, treatment sessions typically range between 30-60 minutes.

  • Do you accept my health insurance?
    • We are participating providers in and accept most major health insurance plans and networks, including Medicare and Medicaid.  We also accept patients who prefer to or need to pay out-of-pocket for their physical therapy services.

  • Will your office bill my health insurance for me?
  • How do I know if my health insurance covers physical therapy services?
    • Our office provides insurance benefit verification service as a courtesy to our patients. However, we also recommend that you directly contact your health insurance carrier as some benefit information is only available to the insured party (patient). For more information, contact our office.

    • It is also important to understand that insurance coverage of physical therapy services is expected only if the services are deemed medically necessary and if there is an expectation for reasonable progress of functional activities. “Maintenance” of your condition is not covered by most health insurances. However, Priority Physical Therapy values ongoing health, wellness, and prevention. Should your condition no longer qualify for coverage by your health insurance, you may consider some of the cash-based services offered by Priority Physical Therapy.   • What if I don’t have health insurance?
    • Priority Physical Therapy offers discounted, cash-based physical therapy, maintenance, and wellness services for those individuals who prefer or require this option. For more information, contact our office.

  • What if your office is not in my health insurance network?
    • While our office does participate in most health insurance networks in northcentral Wisconsin, this situation does occasionally arise. If this should apply to your case, you have a few options:
        • Most health insurance policies offer benefits for out-of-network providers.  You may choose Priority Physical Therapy as an out-of-network provider.
        • Contact your health insurance carrier and/or employer to request that they consider Priority Physical Therapy for their provider network.  They may agree to this on a case-by-case basis, or they may consider granting Priority Physical Therapy longer term in-network-provider status for your case as well as future cases.
        • Inquire about our discounted, cash-based physical therapy, maintenance, and wellness services.

  • Do I need a referral/script/prescription from my physician/provider to see your therapists?
    • In most cases, no. The State of Wisconsin does allow direct access to licensed physical therapists without a referral from another health care provider in nearly all situations. However, some health insurances will require a referral to physical therapy from another qualifying health care provider. Please contact your health insurance carrier or our office for more information on your specific case.

  • Do I need to see my doctor before I start physical therapy?
    • In many cases, no. However, some health insurances do require this in order to qualify for coverage of physical therapy services (see above answer). Also, the providers at Priority Physical Therapy value collaboration and open communication with any and all health care providers relevant to the care of our patients. Given this, your physical therapist may communicate necessary information with your doctor and/or any other providers involved in your case, to assure the best outcome and continuity of your care.

  • How will you access my medical information since you are not part of my hospital/clinic electronic medical record system?
    • Privacy laws and regulations allow for medically necessary communication between health care providers to assure for continuity of care. Your physical therapist may contact your doctor/health care provider’s office for copies of those records or reports relevant to your physical therapy care. In addition, your physical therapist will maintain open communication with your health care providers as appropriate throughout your rehabilitation process, including physical therapy initial evaluation and progress reports. Also, you will personally have an opportunity to provide your physical therapist with a thorough history or background regarding your symptoms, injury, and medical history at your first, initial evaluation visit. You are welcome to bring any relevant records or reports with you to your physical therapy appointments.

  • Do you accept credit cards?
    • Yes, we accept Visa, Mastercard, and Discover, as well as cash and/or checks, for direct payment. We also participate in Care Credit which offers special financing for healthcare services.

  • Why do you need my social security number (or a portion of my social security number)?
    • Personal information, including some or all of their social security number, is required for billing purposes for those patients who do not pay for physical therapy services in full on the date of the service (i.e. for those who prefer we bill their health insurance).
